The Department of Forensic Medicine & Toxicology was started in the year 1918 when Rai Bahadur J. P. Modi was appointed as whole time Lecturer.Modi is known in the entire country for his pristine brilliance and work in the field of Forensic medicine and toxicology. Modi A Textbook of Medical Jurisprudence and Toxicology reigns supreme in its field since its first edition in 1920. It is a well-established, specialized and highly recommended text on Medical Jurisprudence and Toxicology and the most accepted book as reference in the court of law. Most of the cases quoted in the book are from Lucknow and Agra
He later held the post of Materia Medica & Professor till 1934. After his departure the department was managed by part time H. O. D's (Civil Surgeons/ C.M.O.) till 1982.

Prof S. D. Yadav took charge as full time Professor & H. O. D. in 1982, followed by Dr H. C. Rastogi, and Dr.Baljeet Singh.

The Department of Forensic Medicine & Toxicology was the first in India to start awarding P.G degree in the subject in year 1951.

Among the first students who were awarded PG degree was Dr C K Franklin, who later conferred his services as HOD Grant’s Medical College, Mumbai. Among other prominent alumni of this department are Dr Jagdish Chandra, Dr.K.Mathrani , Dr B.B.Aggarwal, and Dr V.K.Jaipalan. Prof Jagdish Chandra started the Department of Forensic Medicine & Toxicology in AIIMS, New Delhi.

The department was housed with the department of Pathology upto 1951 when this dept was shifted in present separate building. The laboratory of this department was shared with Department of Community Medicine upto 1961.

The department is currently headed by Dr Anoop Kr Verma, under whose headship the new building of the department was inaugurated on 21st Feb, 2019 by Shri Ashutosh Tandon, Minister for Medical and Technical Education. It is a two story building built as per NMC requirements for 250 admissions. It has demo rooms which are audio visually well equipped for interactive teaching, and a museum with rich collection of medico legally relevant specimens.
The mortuary to was shifted its present premises in 2001.

The mortuary is well equipped with a cold room (capacity of 18 bodies), all the instruments required for the procedure and autopsy tables.
Along with the main post-mortem room there is a provision of other ancillary areas like consultant’s room, room for mortuary supervisor, attendant’s and cleaner’s room and store rooms. A proper waiting area is present for the relatives of the deceased.
There is a seating arrangement for 50 students in the mortuary room for practical training purposes.
On an average about 6000 post-mortems are done annually.

• Lectures to Additional judges, Civil judges &Prosecuting officers
• Consultation to CBCID of UP .
• Lecturers of PMS doctors regarding various medicolegal problems
• Medicolegal services of various nature to C.B.C.I.D., Judiciary Police, Administration & G.M. & A.H.
• Several textbooks have been written. These include “Text Book of Medical Jurisprudence & Toxicology†written by Dr J.P. Modi Former Prof. & Head Dept of F.M.T., C.S.M.C. in1920 and “Medicolegal Companion†by Dr P.K. Bhattacharya former reader F.M.T. in 1964. “A Short review of Forensic Medicine & Toxicology†was written by Dr Baljeet Singh
• Forth coming year will be witnessing many advancements in the department with starting of Poison Information Centre.
